Community Health Network Area 9 (CHNA9) has announced mini-grant opportunities of up to $2,000 for short-term/limited projects, activities, or events. Proposed projects must address health equity as defined on the CHNA 9 website and the proposal must demonstrate direct impact on the people most directly affected by health disparities. Proposed activities must be located within the CHNA 9 network or the Heywood Healthcare service area. Funding is made available by Heywood Healthcare and CHNA 9 through the Community Health Initiative and is limited to one year.
