The Office of Healthcare Innovation and Quality in Commonwealth Medicine provides consultation and technical assistance to payer and provider organizations in redesigning the healthcare delivery system to achieve the Triple Aim. Our experts support the design and implementation of new models of care delivery, including the Patient Centered Medical Home, Accountable Care Organizations, and the integration of behavioral and physical health care.
Under the general supervision of the Director of Transformation and Quality, the Practice/Health System Transformation Specialist will be responsible for ongoing training, support, encouragement, consultation, and monitoring to practice s and health systems as they strive for clinical and operational improvements through implementing the Patient Centered Medical Home (PCMH) and other new health delivery models. The Transformation Specialist will also assist in the development and implementation of learning collaborative focused on supporting practices and health systems in transforming the way they deliver care.
