Brief Summary of Position:
Under the direction of the Deputy Chief Medical Officer of Commonwealth Medicine (CWM)/Senior Director of the Office of Healthcare Innovation & Quality (HI-Q), this position serves as the lead facilitator and catalyst for improving the clinical and operational performance of clinical practices and provider organizations implementing the Patient Centered Medical Home (PCMH) model of care. The PCMH Quality Improvement Director and Transformation Expert provides leadership in planning, reporting, evaluating, and facilitating the achievement of practice transformation and performance improvement. He/she acts as a liaison between participating practice sites and all stakeholders associated with CWM’s PCMH activities. This person will manage and supervise a team of Transformation Specialists overseeing day-to-day PCMH practice facilitation activities, and will lead the design and implementation of learning collaboratives for practices participating in PCMH and other healthcare delivery reform initiatives. Additionally, the PCMH Quality Improvement Director and Transformation Expert will provide expert advice to PCMH project leaders on the development, communication and implementation of a clear vision and strategy as to how practices will successfully transform into medical homes.
